- Assesses the clinical, infusion related, educational and psychosocial needs of the client and caregiver.
- Designs individualized plans of care with the client and caregiver.
- Promotes an environment of collaboration and team when dealing with the client and caregiver, managing provider, home health agency and other members of the health care team to ensure safe and effective home infusion therapy services.
- Prepares appropriate educational materials for each client and implements clinical and educational interventions based on identified needs and evidence-based guidelines.
- Accurately and as per policy documents in the medical record to adequately reflect assessments, plans of care, interventions, collaboration and evaluation of therapy compliance and effectiveness.
- Collaborates with the physician and other health care professionals to develop home infusion protocols to meet each client's specific infusion therapy needs.
- Is able to respond to the concerns and complaints of clients and their caregivers, physicians, referral sources or home health agencies by utilizing appropriate listening techniques then referring the individual and group to the appropriate resource for resolution of the issue.
- Performs case management and consulting functions with all members of the health care team.
- Cooperates with and shares responsibility with other nursing staff for covering week day, holiday, and weekend on call services to ensure 24 hour a day and 7 day a week coverage.
