ChristianaCare is currently looking for an APC to join our Nutritional Support Team. The selected candidate will work collaboratively with the Medical Director for the Nutritional Support Team as a self-directed practitioner who provides leadership and facilitates coordination of services to maintain/improve the nutritional status of parenterally fed nutrition support patients through the nursing process, which includes ongoing assessment, provision of care, and evaluation of patient response to the care provided. This is a Monday-Friday position with rotating weekend coverage.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

• Performs timely history and physical assessment in response to consults and collaborates with physicians and other members of the medical staff as part of a team approach concerning the achievement of patient's nutritional goals, implementation of care plans, and measurement of outcomes for patients on parenteral nutrition
• Demonstrates the ability to formulate and maintain positive working relationships with peers, managers and other members of the health care team and the Nutrition Therapy Team
• Evaluates the potential for nutrient/pharmacologic interactions in nutrition support patients in coordination with pharmacy
• Prescribes medication and initiates orders as credentialed by the Joint Practice of the Delaware Board of Medical Practice
• Participates in the monitoring of patient response to nutrition therapy including prevention, identification, and correction of potential metabolic, infectious, or technical complications
• Provides and documents patient/family education on home infusion techniques in the medical record and collaborates with the discharge planning staff and/or home agencies to ensure a smooth transition and continuity of care for patients being discharged on nutrition support
• Maintains professional licensure and nutrition support certification; keeps abreast of new developments in the field of nutrition support
• Participates in the identification, development, implementation, monitoring, evaluating, and reporting of performance improvement initiatives
• Participates in development and review of policies, procedures, and guidelines related to nutrition support, administration of formulas, and management of access devices; assists with systemwide communication and implementation of policies, procedures, and nursing guidelines
• Assists with the insertion of catheters/enteral access tubes and teaches insertion techniques to nurses as required
• Demonstrates understanding and application of ChristianaCare's values of excellence and love
• Demonstrates compliance with ChristianaCare's systemwide and departmental policies, procedures, and mandatory education requirements
• Performs related responsibilities as required or directed

EDUCATION AND EXPERIENCE REQUIREMENTS:

• Masters' degree in Advance Practice Nursing or Physician Assistant
• License or license eligible in the state of Delaware
• Prescription Authority required
• Related clinical experience with the adult population preferred
• Nutrition support certification by the National Board of Nutrition Support Certification, Inc. within two years of hire date
• BLS and ACLS required
