Collaborates with the multidisciplinary team to provide quality, efficient, accurate and safe home infusion therapy services in compliance with accrediting bodies. Assesses, plans, implements, coordinates, monitors and evaluates the effectiveness of the infusion plan of care and ongoing educational and associated support needs.
The Intake RN is responsible for reviewing referrals to determine appropriateness of referral.
Gather and review clinicals applicable to requested services to determine medical necessity.
Reviews and compile accurate clinicals to assist in establishing reimbursement by the insurance provider.
Ensures appropriate infusion access device is placed per INS (Infusion Nursing Society) guidelines for ordered therapy.
Communicates care alternatives to referral sources, physicians and clients when a client does not meet admission criteria for home infusion therapy, does not have home infusion benefits or denies home infusion.
Obtains prescriptions and associated lab orders from the provider. Communicates orders and labs to home health agencies.
Works to provide a smooth transition to the home setting.
Provides clinical screening of all infusion therapy patients to establish medical necessity.
Must determine appropriate ICD-10 codes are assigned to patient diagnosis to ensure reimbursement from insurance provider.
The intake nurse must be able to triage phone calls of patients, providers and home health agencies.
Possess the ability to troubleshoot patient issues over the phone to assist in any problems the client may have in the home setting with the ordered infusions.
Nurse must have working knowledge of all pumps and methods of administration to assist patients in the after- hours on call setting.
Will process all forms of orders and make sure the EMR is up to date.
Communicates and works closely with referral sources within and outside the health system.
Provides education to clients and referral sources on Central Venous Access Device (CVAD)
Works with leadership team as directed to review and develop infusion protocols, standards of care, procedures and patient education material.
Assesses the clinical, infusion related, educational and psychosocial needs of the client and caregiver.
Designs individualized plans of care with the client and caregiver.
Promotes an environment of collaboration and team when dealing with the client and caregiver, managing provider, home health agency and other members of the health care team to ensure safe and effective home infusion therapy services.
Prepares appropriate educational materials for each client and implements clinical and educational interventions based on identified needs and evidence-based guidelines.
Accurately and as per policy documents in the medical record to adequately reflect assessments, plans of care, interventions, collaboration and evaluation of therapy compliance and effectiveness.
Collaborates with the physician and other health care professionals to develop home infusion protocols to meet each client's specific infusion therapy needs.
Is able to respond to the concerns and complaints of clients and their caregivers, physicians, referral sources or home health agencies by utilizing appropriate listening techniques then referring the individual and group to the appropriate resource for resolution of the issue.
Performs case management and consulting functions with all members of the health care team.
Cooperates with and shares responsibility with other nursing staff for covering week day, holiday, and weekend on call services to ensure 24 hour a day and 7 day a week coverage.

Monday through Friday from 8:30am to 5pm
Rotation of 1 weekend approx. every 12 weeks; 5 nights in 12 weeks and 1 holiday per year
