Job SummaryWill be a Licensed Practical Nurse performing duties as a patient navigator guiding incoming calls and facilitating entrance of new patients into the program. Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients. The Nurse Navigator, functioning in the spirit of teamwork, is accountable for the delivery of care utilizing skills learned in both the clinic and didactic arena. Assists other team members on the unit.
