The role you'll contribute:
Responsible for ensuring safe and quality hospital-level clinical care and excellent consumer experience for patients. This position collaborates with patients, families, social workers, nurses, providers, and the interdisciplinary team to identify eligible patients. Additionally, this role coordinates all aspects of patient care transfer from the acute care hospital to the patient's home, maintaining patient satisfaction and safety. In addition, this position communicates daily with the interdisciplinary team, responds to plan of care changes, and coordinates in-home resources. Additionally, this role evaluates post-discharge needs, develops transition care plans, and implements post-acute care plans. This position serves as an educational resource for patients, families, providers, and clinical staff.
The value you'll bring to the team:
- Collaborates with Utilization Management staff on patient status changes and medical necessity discussions.
- Educates patients and families about post-acute care services, including home health, infusion services, durable medical equipment, palliative care, hospice, outpatient services, and community-based organizations.
- Reviews patient census and identifies patients meeting inclusion criteria for the Hospital at Home program.
- Assists providers in obtaining informed consent for the Hospital at Home program.
- Facilitates collaborative management of patient care across the continuum to ensure timely, safe, and efficient care delivery.
- Establishes and documents anticipated date of transition and estimated discharge date based on team input, updating as needed.
The expertise and experience you'll need to succeed:
Minimum qualifications:
- Associate's of Nursing
- 2 years of acute care nursing experience
- Registered Nurse State Licensure and/or Compact State Licensure In Florida
- Active American Heart Association BLS
- Valid in state Drivers License with current car insurance
Preferred qualifications:
- Bachelor's of Nursing
- Masters of Nursing
